Business Visa Application

Requirement :
1. Application Letter
2. Copy of Invitation Letter from Saudi Arabia/Pakistan
3. Copy of applicant passport
4. Copy of Form 9, Form 24 & Form 49.

Charges per person : RM45 (Malaysian Passport) / RM70 (Non-Malaysian Passport)

Tata Syarat :
1. Surat permohonan syarikat
2. Salinan surat undangan dari Arab Saudi/Pakistan
3. Salinan paspot pemohon
4. Salinan borang 9, borang 24 & borang 49.

Caj bagi setiap orang : RM45 (Passport Malaysia) / RM70 (Passport lain-lain negara).

Cara-cara memohon Sijil Tempasal

SIJIL TEMPASAL (Certificate of Origin)
- Satu sijil pengesahan tentang tempat asal sesuatu produk yang dihasilkan dan memerincikan maklumat produk yang ingin dieksport melalui pengisytiharan di dalam satu format borang yang telah ditetapkan oleh badan-badan dan agensi-agensi yang terlantik.

Dokumen-dokumen yang diperlukan bagi memohon pengesahan Sijil Tempasal tersebut telah dibahagikan kepada 2 peringkat adalah seperti berikut :

Peringkat pertama : Kelayakan

1. Surat Jaminan (Letter of Indemnity) mengikut format yang telah disediakan dimana hendaklah dihantar seminggu sebelum mendapatkan pengesahan sijil tempasal
2. Surat Akuan Berkanun mengikut format yang telah disediakan.
3. Salinan Pendaftaran Syarikat (Borang 9) atau Pendaftaran Perniagaan (Borang D), Borang 24 & Borang 49.
4. Lesen Pengilangan
5. Borang Analisis Kos - jika perlu.

Peringkat kedua : Pengesahan

1. Borang Sijil Tempasal yang lengkap diisi
2. Invois Perdagangan
3. Invois Pembekal jika ada
4. Packing List
5. Bill of Lading atau Air Way Bill
6. Borang Pengisytiharan Kastam (K2) dan resit
7. Dokumen-dokumen lain yang berkaitan bagi tujuan eksport dan eksport semula dari Malaysia apabila diperlukan oleh DPMMS.

How to apply Certificate of Origin

WHAT IS CERTIFICATE OF ORIGIN (COO) ?

A document that certifies the country of origin of a particular product and it is a declaration document made by the exporter. Click here to see a Work flow for applying Certificate of Origin.


2 stages to be done for applying Certificate of Origin.


FIRST STAGE : Qualifying

(MUST be submitted 1 week before getting the COO endorsement and valid for 1 year)

1. Letter of Indemnity which type under company letterhead

2. Statutory Declaration

3. Copy of Company Registration (Form 9) or Business Registration (Form D), Form 24 & Form 49

4. Manufacturing License

5. Analysis Cost of Product (If any)

SECOND STAGE : Endorsing

1. Certificate of Origin Form

2. Commercial Invoice & supplier invoice

3. Packing List

4. Bill of Lading

6. Custom Declaration Form (K2)

7. Other related documents in respect of exports and re-exports from Malaysia (export declaration) that required by the DPMMS.

* Documents must be produced additionally as a copy for DPMMS retention.

What is Certificate of Origin

A Certificate of Origin (CO) is a document attesting that goods in a particular export shipment are wholly obtained or produced or manufactured or processed in a particular country (country of origin). Virtually every country in the world considers the origin of imported goods when determining what duty will be assessed on the goods or, in some cases, whether the goods may be legally imported at all.
